March 2021 Sponsorship Request
To request a custom hospital gown for your child, please fill out the form below (if you do not see a form asking for your child's name, date of birth, etc - you need to switch to the web version of our website). Please remember to include information on what your child enjoys, so we can better personalize their gown.
Completion of your child's hospital gown can take up to 8 weeks (or more depending on how long it takes for them to be sponsored)from the date of submission, due to needing to post your child's information and photo (which you will need to e-mail to jessica@graciesgowns.org with your confirmation number) to our sponsorship page, and wait for your child's gown request to be sponsored.
ONLY (1) gown per child will be fulfilled through the sponsorship - if multiples are needed/wanted, you will need to visit the in stock gowns, or request a custom one and cover the costs.
Rush fees can be added to expedite your child's gown within 4 weeks from the date of submission - but will not be covered by sponsorship.
